If your GP or specialist has booked you straight in for a procedure without a consultation first, this is called direct access. Tell us where you're going and what you're having, then read your instructions and complete your forms online.

Reception fills in your procedure date and time, and ticks which instructions to follow. If we sent you a personal link, please open that link instead.
Complete your forms
This replaces the paper patient details form. Fields marked * are required. Nothing is sent until you choose to send it. Your answers stay in this browser tab.

- Email your GP referral letter to reception@chatswoodendoscopy.com.au within seven days, or fax it to (02) 8964 8664.
- Email proof of your gap payment to reception@chatswoodendoscopy.com.au within seven days.
- Complete the hospital's online admission. The link is in your booking letter above.
